Writing Effective Findings
Audit findings should clearly explain the issue, supporting evidence, associated risks, and recommended corrective actions. Clear documentation enables management to understand and address identified problems.
- Condition: What was found?
- Criteria: What should exist?
- Cause: Why did the issue occur?
- Effect: What is the risk or impact?
- Recommendation: How should it be corrected?
